Athletics Participation Policy
INFORMATION REGARDING PARTICIPATION IN HIGH SCHOOL ATHLETICS

ACADEMIC ELIGIBILITY REQUIREMENTS Students must have earned a minimum of twenty (20) credits and have achieved at least a 2.0 Grade Point Average on the last report card. These standards must be maintained during the season of competition. Eligibility is determined on the mid-term and final report cards. These dates and requirements are determined by the Los Angeles Unified School District.

REQUIRED INFORMATION The following must be completed and on file before any student is allowed to participate in athletics or take part in after-school practice.

Physical Examination: Must be done by a private physician or at one of the many local clinics. The physician or clinic in addition to the physician�s signature must stamp the exam. Physicals are good for only one year.

After the physical is completed the following will be issued:
  • Interim Health Information: Must be completed for each sport.


  • Proof of Insurance: Supplemental insurance is available through the school at the student's expense. A photocopy of the insurance must accompany the insurance card.


  • Athletic Record Card: Complete the top portion of the card and sign


  • Emergency Information Card: Must be completed with signatures and insurance carrier
