Athletic Assistants

Note:Newly Added Certificates: Suicide Prevention Course (District required - on MyPLN) will be available online and coaches will receive an email notification once the course is available online.  Certificated coaches that took the course online last Spring will not have to retake the course this fall.  Athletic assistants will need to take the Suicide Prevention course beginning this Fall.  CIF State is adding Heat Prevention Illness course for coaches and beginning with winter coaches it will be mandatory to take this course on
Athletic Assistants-
1-New coaches will need: Drivers license or government issued ID, Social Security Card. 
2- Fill out Athletic Assistant Coaches' Application - Click Link # 2 below (Scan and email via PDF to
3-Coaches will need a TB test. The test results will be read 48—72 hours after the initial test. If the test reads positive, you MUST have a chest X-ray. Please note that you must provide documentation that your skin test read positive.T.B. skin tests and chest x-rays must be taken and read within 60 days prior to your processing appointment. Make sure that you receive an original document showing the NEGATIVE TB results; you will need to submit the results at the processing appointment.
4- AD's will send information to the District. 5 days later, a Tracking number will be issued. 
Note: Starting August 2018- All classified coaches must fill a Freeze Form, Athletic Assistant Form and Nepotism for before starting any coaching assignment. The forms are found at the bottom of the page. 
5- Office Secretary or AD will set up an appointment to process fingerprints- 
  1. Drivers license or Government issue photo ID. (passport or ID)
  2. Mantoux TB test, administered within 60 days of processing
  3. Social Security Card (must have the same name on SS card and driver's license)
  4. Completed Request for Personnel Action Form (RPA)-Receive from office secretary- Jackie S. 
  5. Nepotism Form (Receive from office secretary- Jackie S.)
  6. Must be 21 years of age
  7. Proof of First Aid/CPR and AED from American Red Cross or American Heart Association.  
There are two locations for Fingerprinting:
Valley Employment Office- 818-654-1600, 6505 Zelzah ave, Building 6, Reseda, Ca. 91335. Offices are open 8am to 4:30pm, M-F 
Mid Cities Employment Office- 323 753-3321 (press 9) 944 W. 77th. Los Angeles, Ca. 90044. Offices open 8am to 4:30pm. 
6- Coach will notify Athletic Director of a successful visit. May take up to 3 weeks to clear. 
7- Take and pass Coaching Education Class. Each Test has a fee (non reimbursable)

8- Pass Sudden Cardiac Arrest course (good for 2 years):
9- Pass Concussion course (good for 2 years):
10-(NEW)Heat Illness Prevention Course (good for 2 years)
11- Get CPR and First Aid certified (non-online course) Must be Red Cross or American Heart Association approved.(Good for 2 years)
12. Once LAUSD approves background and fingerprints, a employee # will be issued. 
13. Using LAUSD employee #, Coach must pass District Child Abuse training:
 14- Issue all certificates to Athletic Director via Email ( and in PDF format.

16-Suicide Prevention Training- See Memo for instructions.
Swim Coaches-Swimming Water safety exam ( good for 2 years) 
Cheer Coaches- AACCA Cheer Course ( good for 4 years):
