Volunteer Coaches

 
 
1-New coaches will need: Drivers license or government issued ID, Social Security Card.  ( Names must match)
 
2- Fill out VOLUNTEER coach form - Click below in links (Scan and email via PDF to Mag7521@lausd.net)
 
3-Coaches will need a TB test. The test results will be read 48—72 hours after the initial test. If the test reads positive, you MUST have a chest X-ray. Please note that you must provide documentation that your skin test read positive.T.B. skin tests and chest x-rays must be taken and read within 60 days prior to your processing appointment. Make sure that you receive an original document showing the NEGATIVE TB results; you will need to submit the results at the processing appointment.
 
4-Set up an appointment to process fingerprints- 
  Coaches must take- Government issued ID, Social Security Card and Negative TB results. There are multiple  locations for fingerprinting. Ask our office secretary, Jackie Sirounian to make the appointment for you.

6- Coach will notify Athletic Director of a successful visit. May take up to 3 weeks to clear. 
 
7- Take and pass Coaching Education Class. Each Test has a fee (non reimbursable)
There are two options- A) ASAO or NHFS.

11- Get CPR and First Aid certified (non-online course) Must be Red Cross or American Heart Association approved. 
 
12- Code of Conduct form- Read and Sign ( see below for link)
 
 13- Issue all certificates to Athletic Director
 
Note-Classified or Certificated staff do not require re-finger-printing.
