Athletic Clearance for New Athletes to Grant High School
As a new athlete to Grant High you need to create a Home Campus account, the link is provided below but first, you will need the following information:
- 1.Parent's email address
- 2.Home address, home and cell phone numbers
- 3.Emergency contact information
- 4.Medical information, including medical insurance information, doctor's name and phone number(s).
- 5.Digital copy of medical insurance card. A picture can be uploaded into the account.
- 6.You will need access to a printer to print the Parent Consent Letter. If you don't have a printer email it to yourself and print it at school.
Important reminder- The account must be under the parent's name and email address.
Please print the final Parent Consent letter, make a copy of it, and give it the student's coach.
